Showing a modal dialog and leaving screen or region behind it as it is, may give feel that still that screen behind is usable though dialog is modal.<br /><br />If used like a shadow effect for entire screen behind modal dialog may help user and makes easier to guess that dialog has to be answered. A kind of user experience test idea. <br /><br />I see title bar of dialog in web helps in contexts I can think of and also it can be little freaky in other contexts I'm thinking now.
